Complex care is a higher level of care than what is provided by long-term care homes and other nursing facilities. These medically-complex patients have chronic illnesses or disabilities, and they typically require 24-hour nursing care and access to hospital services. Complex care is a specialized, time-limited program providing patients with ongoing onsite assessment and active care with a goal to enhance the health and quality of life. The average length of stay is 45.1 days. Complex care beds are located at Douglas Memorial, Greater Niagara General, Port Colborne, Niagara-on-the-Lake and Welland sites. All patients/clients and families are encouraged to be active members of the health care team. The team includes physicians, nursing, social work, case management, a clinical pharmacist and clinical dietitian as well as programs for ventilator care, occupational therapy, physiotherapy, speech therapy, therapeutic recreation, chiropody services, spiritual/religious, palliative care and oral hygiene.
At the Welland Site, a dental clinic is open in which most dental procedures can be provided for elderly and disabled clients. A partnership has been established with Niagara College to provide these preventive and oral hygiene services.
