Complex Continuing Care (Chronic Care)
Inpatient Complex Continuing Care beds are located at Douglas Memorial, Greater Niagara General, Port Colborne General, Niagara-on-the-Lake and Welland Hospital Sites. The CCC units offer a range of hospital-based services to clients with complex or chronic conditions requiring skilled and continuing interdisciplinary care and professional medical management. All clients and families are encouraged to be active members of the health care team. The team includes physicians, nursing, social work, case management, a clinical pharmacist and clinical dietitian as well as programs for ventilator care, occupational therapy, physiotherapy, speech therapy, therapeutic recreation, chiropody services, spiritual/religious, palliative care and oral hygiene.
At the Welland Hospital Site, a dental clinic is open in which most dental procedures can be provided for elderly and disabled clients. A partnership has been established with Niagara College to provide these preventive and oral hygiene services.
