Mel Clarke says Niagara Health's Integrated Comprehensive Care (ICC) program has changed his life.
The Niagara Falls resident, who has Chronic Obstructive Pulmonary Disease, went through the ICC program last fall after being discharged from hospital. The program provides discharged patients with access to care in the community to help manage their chronic condition at home.
