Community support and partnerships

As an active partner in the Niagara Ontario Health Team – Équipe Santé Ontario Niagara webpage, opens in new tab, Niagara Health is helping advance integrated models of care that better connect patients to the right services at the right time. This includes programs such as SCOPE Niagara celebrates four years, opens in new tab, which supports primary care providers with rapid access to specialist advice; Let’s Go Home program helps Niagara residents transition safely from hospital to home, opens in new tab, which helps patients transition safely from hospital to home with appropriate supports; and remote care monitoring initiatives that enable patients to receive care and follow-up in the comfort of their own homes.

In addition, we established important new partnerships with Niagara Region to support physician recruitment and strengthen our Emergency Department teams. While we continue to build new relationships across the healthcare system, we are equally proud of the longstanding partnerships that have grown stronger over time and continue to make a meaningful difference in the lives of patients and families throughout Niagara.
