Awards of Excellence Nomination

The Awards of Excellence were developed as an opportunity to recognize extraordinary service within Niagara Health (NH). Selection for each of the nine award categories is based on the NH purpose, vision, CORE values, and areas of focus. While it is recognized that contributions of outstanding performance can be made across all categories, award selection will be based on significant overall evidence in one of these areas. The President and CEO Award of Extraordinary Performance will be presented to the individual or team making the most significant impact overall.

All nominations will be considered for the President and CEO Award of Extraordinary Performance.

Terms

Anyone can nominate (employees, volunteers, physicians, students, customers etc.)
Teams can self-nominate (program teams, committees, project teams, etc.)
Individuals cannot self-nominate for the individual awards
Individuals and teams may be nominated more than once
Individuals and teams may receive an award for one award category only, however, an individual winner may receive an additional award if they are also a member of an award winning team
Nominations are submitted to Organizational Development at the Welland Site
Nominations are accepted year round. Nominations received after the nomination deadline date will be considered for the following year
All nominations will be considered for the President and CEO Award of Extraordinary Performance.
Awards may not necessarily be presented in all categories every year.
All nominations must include a full description of the situation, the action that was taken and the outcome. See below for a more complete description of the requirements of the nomination.
Nominations are limited to a maximum of 250 words.
All nominations must also include a brief summary of the nomination in 80 words or less to be shared with the nominee.
All nominations must be endorsed by the employees manager.
