On July 23, 2020, the Minister of Health and Deputy Premier, Honourable Christine Elliott, announced that Niagara has received approval to move forward as an approved Ontario Health Team. Niagara Health is one of 45 partners who comprise the Niagara Ontario Health Team - Équipe Santé Ontario Niagara (NOHT-ESON).
The NOHT-ESON partners provide a wide array of services, which include primary care, home care, emergency services, public health, social services, mental health and addictions, rehab and acute care. We’ll all work together as one team to create a coordinated approach to care that makes it easier for patients to access services, creating seamless transitions between providers, and overall improves patients’ experience and health outcomes. The NOHT-ESON will also focus on building trusting relationships with the Indigenous and francophone population.
In the short term, how we provide care for Niagara Health patients will not change, and Niagara residents will continue to contact their healthcare providers as they do now. Transitioning to this new model of partnership and seamless care will take time, and we will keep you informed of developments in this work.
Congratulations to our partners. We look forward to working together to implement this new model of care.
