A design that strengthens social connection and processes within and across staff, physicians, patients and community partners. A design whereby patients, families and caregivers have access to community-based services, and supports seamless transitions within the community.

Here are a few ways we are working towards our goal:
  • Working with the Niagara Ontario Health Team (OHT) and partners to strengthen and streamline community resources and reserving hospital visits for urgent and chronic conditions.
  • Consulting and incorporating lessons learned, feedback and insights received from NH staff and physicians, Niagara Health (NH) Engagement Network (patients and families) and community partners.
  • Auditorium and outdoor space for gatherings
  • Inclusion of works by local artists
  • Multiple access points that will include a connected pedestrian and bicycle path network as well as an on-site public transit hub
  • Improved integration of technology including, 100% wireless infrastructure, nurse call system, NH Navigator and Virtual Care integrations

Did You Know? Research shows us that maintaining social connections has many health benefits from enhanced mood to lower blood pressure. The SNS will have several indoor and outdoor accessible spaces for patients, family and staff to mobilize and socialize, such as, the auditorium, gardens and terraces and trails.
